Case Manager

 

Major Duties and Responsibilities

  • Provides education, appropriate referrals and services to caseload of 60 at-risk families.
  • Helps at-risk families take all steps needed for safe delivery and healthy babies.
  • Assesses the educational, medical and social needs of clients who are at-risk for poor pregnancy outcomes.
  • Conducts home visits, well-women visits, post-partum visits, and provides health support to individual children and families, as needed.
  • Initiates provider tracking for clients to ensure clients’ access to medical care and referrals for services.
  • Works with staff and provides technical assistance, including proper procedures for the prevention of the spread of illness, assessment of safety concerns and hazards and other topics as needed.
  • Maintains/updates client files and records
  • Performs other duties as assigned.

 

Education:

  • Registered Nurse (RN) Licensure required, Bachelor’s degree, preferred.
  • Obtained a minimum of 9 ECE credits.

 

Experience:

  • Minimum of one year of nursing experience, preferred
  • Experience in care coordination and supervising, preferred.
  • Experience in obstetrics and/or pediatric health preferred.
  • Experience in conducting health education and training, preferred.
  • Knowledge of family and systems theory.
  • Knowledge of obstetrics, pediatric health, and health care systems preferred.

 

Other:

  • Must have valid Illinois Driver’s License and insurable by agency’s insurance carrier.
  • Ability to travel locally up to 60%.
  • Ability to work evenings and weekends as needed. 
  • Must be able to transport oneself to and from home visits.
  • At times may be required to lift up to 25 lbs. for various event needs.
